Medical Retina and Wet AMD Additional Clinical Questions

Where the clinic type selected is Medical Retina, Wet AMD or WGOS 4 MR then there is a need for a number of additional clinical fields to be completed. Not all of the fields are mandatory but those that are mandatory have a red asterix adjacent to them to make clear that these need to be compelted.
