Cataract Additional Clinical Questions

Where you are making a referral for cataract surgery, in addition to the general questions and text boxes on the WGOS referral from there are some additional questions as below:

Where the response to Is patient already under the care of Ophthalmology is Yes, then an additional text box is required with the details of their current ophthalmology care.

You will note that there are three options in relation to the pre-operative questionnaire:

Yes - I will upload - this opens an upload field for you to upload the PDF questionnaire

Yes - Record Responses (see below) where the additional pre-cataract questions appear and can be completed
